Just when the screen blacks out when Meadow walks in and Tony looks up,

If you press pause at the right time, you see on the left of the screen the muzzle of a gun.

TONY IS DEAD!!!!!!!

This tied in with Tony's belief that when you die, you don't even hear it. Things go black. Explaining the ending. RIP Tony.
