Check throttle plate for carbon build-up. Have also seen water in fuel rail, stuck O2 sensor, or clogged exhaust causing this. Does the idle seem to dip then 'hunt' for baseline when you first put it in gear or does it just seem 'lopey' at all times?
And yes the truck is 'lopey', seems almost like timing is off, as a matter of fact the engine pings and is very nticable between 1500/200 rpm, unless i have some other serious mechanical issue i don't think it is piston slap?.
