Part Number for Jack Tool Kit?

I bought my 2003 H2 SUV back in 2005 (I bought it used) and have not had any need to use the jack or the spare tire. Lucky for me!!! Why? Because I only just discovered that the previous owner removed/lost the tool kit. All I have is the scissor jack, but no handles to connect to the jack to make it go up or down. I figure the previous owner removed the tool kit when he relocated the spare tire from the inside of the truck to the outside. So all I have is the following:
1. Spare tire
2. Jack
3. Very small black pouch with:
a. air hose
b. various fittings for the air hose
c. wheel lock key

I just recently had to buy the front hitch extender and pin because I knew I was missing that, but because the jack was mounted behind the rear seat, I thought the handle would be with it. That was a very dumb and risky assumption. So, does anyone know the part numbers for whatever I am missing, especially the jack handle? Thanks!!
