Considering the need to be close to good medical care but a less expensive cost of living I would suggest Cleveland, or its outlying areas. The Cleveland clinic has an extraordinary reputation but the city is cheap. Morgantown, WV might be another place with reasonable access to Pittsburgh hospitals and a low cost of living.
